Ending Supervision

At times it becomes necessary to end supervision due to a change in employment, health reasons, moving from a particular area, etc. 

Please contact the Board office to make sure that your file is up to date with all quarterly reports.  We will also need a completed demonstrable competency form. 

Demonstrable Competency Form

Required also - a letter from the supervisee and/or supervisor stating that supervision ended as of whatever date.  This is needed in order to "freeze" the date for taking and passing the EPPP. 

The supervisee may choose to reactivate the licensure process at a future time.
